Due to the overwhelming demand for bus parking we must now charge a fee per bus. This fee will be due at the time of bus registration. This is a “per bus” charge and will only amount to $1 - $2 per rider depending on bus size.

Fee can be paid by check (WFL, P.O. Box 225044, San Francisco, CA 94122) or via PayPal or credit/debit card. (Please use the clickable PayPal buttons on the next page or the link in your confirmation email so we can credit correctly.)

The deadline for registering all buses is available when all details are loaded.

As each bus will require a separate bus captain on the day of the event, we require that you complete this form for each bus that you will be bringing.

Bus Captain Contractually Agrees to the Following

I have not and will not register as bus captain for more than one bus. *
I agree to pay the registration fee as specified. Refund is available if cancellation is received by the January 1 before the Walk. Please make check payable to Walk for Life West Coast and send to: P.O. Box 225044, San Francisco, CA 94122. *
I agree to respond within 24 hours to the Confirmation Notice that will be emailed to me at a later time. *
I agree to obtain a mobile phone number or emergency contact phone number for each of my riders. The completed list will be made available to SFPD on the day of the walk, upon request. *
I agree to personally read and follow all instructions forwarded to me by email. *
I will not instruct the bus driver to park in any area other than the assigned parking area. *
I will not instruct the bus to leave the assigned parking area until ALL riders are back on board. *
I understand that failing to follow through on any rules and or instructions may jeopardize my group from being allowed to bring a bus to this event in the future. *
